The Immunization Supplement asks questions about health and immunization history. This supplement was fielded in September from 1978 through 1985. Types of immunization covered include flu, polio, DTP (diphtheria, tetanus, and pertussis), measles, and mumps. Questions about diabetes, and chronic heart, lung, and kidney diseases are included in the health history portion of the survey.

From 1978-1984, this supplement was asked of respondents in rotation groups 2-4 and 6-8 only. In 1985 all rotations were included. All eligible respondents were asked about receiving a flu shot within the last year. Persons 14 years of age and older were asked about having histories of chronic heart, lung, and kidney diseases. Persons aged 19 and younger were asked about history of, and immunization against, diseases such as mumps, multiple types of measles, and polio. These questions included ever having diseases, having diseases within the past year, receiving immunizations, receiving immunizations within the last year, and number of immunizations received. Licensed daycare enrollment was also recorded for children (see IMDAYCARE for universe age changes).
